This position requires flexibility of hours. Please include a cover letter when applying.EDUCATION:Master's degree from an accredited university in social work, psychology, counseling, rehabilitation, gerontology, sociology or a related field contingent on approval from oversight agency.EXPERIENCE/TRAINING:Minimum 2 years experience working with older adults and/or adults with disabilities required. Experience with care management and work in home settings strongly preferred.SKILLS:Clinical assessment skills, knowledge of community resources and public benefits, ability to work with a multidisciplinary team and with clients/families in the home setting and by telephone.Knowledge of aging programs for seniors and/or adults with disabilities.Computer skills including Microsoft Office or equivalent and ability to learn and utilize specialized software with training and support.Worker Type:RegularFull timeShift:Days
